You must include:
- A certified copy of the student's birth certificate
- Current Proof of Residency, at the address indicated on the Enrolment Form, by way of either a current Lease Agreement, or a current Rates Notice plus one other utility bill (eg. Electricity, Telephone etc. showing the same address).
